Who might be able to join this trial:
- Adults over 18 years old in Thailand who are being treated in a medical intensive care unit (ICU) and have been diagnosed with sepsis or septic shock (a serious infection that is causing dangerously low blood pressure), with the shock starting less than 24 hours before enrolment
- People who are already on a breathing machine or for whom a breathing machine is being planned
- People who already have a special monitoring line placed in an artery, or for whom one is being planned, to continuously track blood pressure
- People whose heart pumping strength, measured by an ultrasound scan of the heart, is above a certain level (confirm exact threshold with trial site)
Who may not be able to join:
- People experiencing dangerous heart rhythm problems, specifically ventricular tachycardia or ventricular fibrillation, at the time of enrolment
- People who have recently had a stroke or bleeding inside the brain
- People who are having a heart attack or acute coronary event
- People whose heart itself is the primary cause of shock, or who are experiencing acute heart failure
- People having a severe asthma attack
- People whose lungs are not able to take in enough oxygen (confirm specific threshold with trial site)
- People with serious, life-threatening bleeding in the digestive tract
- People who are pregnant
- People who need emergency surgery within 2 hours of being enrolled in the trial
- People with advanced-stage cancer where survival is expected to be less than 6 months
- People with low urine output due to kidney problems combined with signs of excess fluid in the body
