Who might be able to join this trial:
- You must be able to understand and agree to take part in the study on your own (have the mental capacity to give consent)

- You must have been diagnosed with COPD (a lung condition) by a lung specialist at West China Hospital (or similar designated hospital)
- You must be 40 years old or older

- It is preferred (but not required) that you have had at least one COPD flare-up in the past 12 months that needed steroids, antibiotics, or an urgent medical visit
Who may not be able to join:
- You have had a COPD flare-up in the 4 weeks before joining the study
- You have also been diagnosed with asthma by a lung specialist at the study hospital
- You have other serious health conditions that might make it difficult to take part, or you have a terminal illness (confirm with trial site)
- You are unwilling or unable to download and use an app called "Epic Care Companion" on a suitable mobile device
- You are currently pregnant
