Who might be able to join this trial:

• People who have been diagnosed with asthma for at least one year
• People aged 13 to 75 years old
• People who have been prescribed an inhaled corticosteroid (a type of asthma preventer inhaler), whether used regularly or on a scheduled as-needed basis — this includes inhalers that combine a preventer with one or two other controller medicines
• People whose asthma is not well controlled (based on a standard asthma symptom score below 20), OR people who needed steroid tablets or injections for 3 or more days, or a hospital stay of at least 24 hours, due to an asthma flare-up that happened between 30 and 365 days before joining the study
• People who are able to give consent (or assent, for teenagers) in English or Spanish
• People who also have a diagnosis of COPD may still be considered eligible if they have never smoked and do not have another lung condition causing airflow problems, OR if they currently smoke or used to smoke and have airflow limitation on breathing tests but a normal lung gas-transfer result from within the past two years (confirm with trial site)
• People taking certain medicines that could interact with azithromycin (one of the study drugs) may still be considered, provided they agree to a heart rhythm test before the study begins and again after one week if assigned to receive azithromycin (confirm with trial site)

Who may not be able to join:

• People who live in the same household as someone already enrolled in this study
• People whose doctor estimates their life expectancy is less than one year
• People who have not been prescribed an inhaled corticosteroid preventer inhaler
• People currently receiving treatment for blood cancer or solid organ cancer, except basal cell skin cancer or squamous cell skin cancer — though people who finished their original cancer treatment more than 12 months ago and may now be on a cancer maintenance medicine might still be considered eligible (confirm with trial site)
• People who are allergic to a class of antibiotics called macrolides, or who have certain health conditions that could make taking this type of antibiotic risky — including active or chronic liver disease, severe kidney disease, low potassium or magnesium levels that have not been corrected, a significantly slow heart rate, or a history of a specific type of abnormal heart rhythm
• People who take oral steroids (steroid tablets) every day or every other day for any reason
• People who have taken a course of steroid tablets for an asthma flare-up, or who were admitted to hospital overnight for asthma, within the past month — eligibility may be re-checked after one month has passed
• People currently taking a specific type of inhaled steroid called R-ICS, or people currently on an antibiotic course expected to last more than 30 days — people on a shorter antibiotic course may be considered after stopping antibiotics for at least 72 hours; people on biologic medicines may be considered if they have been on a stable dose for more than 6 months and meet the other study criteria
• People taking any medicine known to carry a high risk of causing a serious abnormal heart rhythm — this is an absolute reason for being unable to join (a full list of these medicines is held by the trial site)
• People taking any of the following medicines, which require careful monitoring when taken alongside macrolide antibiotics: digoxin, warfarin, theophylline, ergotamine, dihydroergotamine, cyclosporine, hexobarbital, phenytoin, or nelfinavir
