Who might be able to join this trial:
- People who are 40 years of age or older when they arrive at the emergency department.
- People who have been clinically diagnosed with COPD and are visiting the emergency department due to a moderate or severe COPD flare-up.
- A moderate flare-up means a sudden worsening of COPD that requires steroid medication (given by injection, drip, or by mouth) and/or antibiotics.
- A severe flare-up means a sudden worsening of COPD that requires a hospital stay, observation for more than 24 hours in an emergency or urgent care setting, or that results in death.
- People currently using two or three long-acting inhaler medications in combination (such as combinations known as LABA/LAMA, LABA/ICS, LAMA/ICS, or LABA/LAMA/ICS) (confirm with trial site if unsure which category an inhaler falls into).
Who may not be able to join:
- People who currently use only a single long-acting inhaler medication on its own.
- People who leave the hospital against medical advice, or who pass away during their hospital stay.
- People who have a tracheostomy (a surgically created breathing opening in the throat).
- People who have been diagnosed with advanced cancer.
- People who have previously received a lung transplant.
- People whose discharge plan includes transfer to hospice care.
- People who are being transferred to another hospital.

To assess the effect of a digital health tool implementation on patient follow up with a respiratory specialist within 30 days after discharge for an acute COPD exacerbation resulting in an emergency department visit or hospitalization.; To assess the effect of a digital health tool implementation on time to respiratory specialist follow-up after an acute COPD exacerbation resulting in an emergency department visit or hospitalization.
