Who might be able to join this trial:
- People aged 18 years or older.
- People who have been diagnosed with an acute ischemic stroke (a stroke caused by a blockage in a blood vessel in the brain).
- People whose brain scans (CT or MRI with blood vessel imaging) show a blockage in a specific type of smaller blood vessel in the brain, as confirmed by the trial team.
- People whose stroke symptoms score 6 or higher on a standard medical stroke severity scale called the NIHSS.
- People whose treatment can begin either within 6 hours of when they were last known to be well, or between 6 and 24 hours of when they were last known to be well if brain scans show there is still salvageable brain tissue (confirm with trial site for specific scan criteria).
- People who are able to give signed consent to participate.
Who may not be able to join:
- People who have evidence of bleeding in the brain.
- People who had some level of physical disability before the stroke (confirm with trial site for specific scoring details).
- People whose stroke symptoms are improving quickly, to the point where the medical team expects their severity score to drop below 6 by the time of enrolment.
- People for whom the medical team believes the procedure cannot be completed safely or successfully.
- People whose stroke may be caused by blood vessel inflammation, infection-related clots, or a heart valve infection.
- People whose stroke may be caused by a torn artery wall.
- People with a severe allergy to contrast dye used in medical imaging, or for whom contrast dye is not safe to use.
- People with a known condition that affects blood clotting, either inherited or acquired.
- People with a clotting test result (INR) above 1.7, or who have taken certain blood-thinning medications within 48 hours before their stroke symptoms began.
- People with a very low platelet count (below 50×10⁹/L), which affects the blood's ability to clot.
- People with any active or recent bleeding (such as in the stomach or urinary tract), or who have had surgery or a biopsy on an internal organ within the past month.
- People with very high blood pressure that cannot be brought down with treatment.
- People with severely reduced kidney function, based on specific blood or kidney test results.
- People whose brain scans show a large mass effect or a brain tumour (except for a small, specific type called a meningioma).
- People who are not expected to live longer than 6 months due to a serious existing illness, such as advanced cancer or severe heart or lung disease.
- People who are pregnant or breastfeeding.
- People who are already participating in another clinical trial.
- People who, in the medical team's judgement, have any condition that would make participation unsafe or unsuitable, including difficulty understanding or following study procedures due to psychiatric, cognitive, or emotional conditions.
