Who might be able to join this trial:

- You have taken part in another clinical trial for a drug or medical device in the past 3 months, or are currently in one
- You have used Evolocumab or a similar type of medication (called a PCSK9 inhibitor) in the past 6 months
- Doctors estimate your life expectancy is less than 12 months, or there is another reason you may not be able to complete the study
- You have a mental health, memory, or emotional condition that makes it difficult to understand or follow the study's instructions
- You do not wish to take part in the enrollment survey
