  • You have used Evolocumab or a similar type of medication (called a PCSK9 inhibitor) in the past 6 months
  • Doctors estimate your life expectancy is less than 12 months, or there is another reason you may not be able to complete the study
  • You have a mental health, memory, or emotional condition that makes it difficult to understand or follow the study's instructions
  • You do not wish to take part in the enrollment survey
