Who might be able to join this trial:

Care Partners and People Living with Dementia (as a pair):

  • People who provide care for someone diagnosed with moderate to severe dementia
  • People who are able to give their own informed consent to participate

Home Health Care Professionals – Care Managers and Field Nurses:

  • Care managers who regularly help with the process of moving patients with dementia into hospice care
  • People aged 18 or older

Home Health Care Professionals – Medical Providers:

  • Doctors or nurse practitioners who refer patients for hospice care enrollment
  • People aged 18 or older

Home Health Care Administrators:

  • Administrators who work within a Certified Home Health Agency or an Advanced Illness Management Program that refers patients to hospice care
  • People aged 18 or older

Who may not be able to join:

Care Partners and People Living with Dementia:

  • People under the age of 18
  • Care partners who are caring for someone with only mild cognitive impairment (rather than moderate to severe dementia)
  • People living with dementia whose condition is classified as mild cognitive impairment only

Home Health Care Professionals (Care Managers, Medical Providers, and Administrators):

  • Professionals who do not have experience managing the transition of dementia patients into hospice care
