Who might be able to join this trial:

Clinics:

• The clinic must be an active participant in the St. Louis Integrated Health Center Network for Community Academic Partnerships.
• The clinic must employ at least 3 clinicians who regularly manage adult asthma patients (meaning they see at least one adult asthma patient per week on average).
• The clinic must share data with a system called Azara for reporting purposes.
• The clinic must be willing and able to take part in all parts of the SMART program, which includes educational visits, ongoing coaching and support, regular review of patient outcomes and feedback, patient education materials, asthma action plans, and monthly meetings.

Doctors and clinical staff:

• The person must have signed and dated a consent form agreeing to take part.
• The person must hold a current state licence as a physician, assistant physician, nurse practitioner, or physician assistant.
• The person must regularly care for adults who have asthma (this may include those trained in family medicine, internal medicine, obstetrics/gynecology, or specialist medicine).
• The person must be willing and able to incorporate the SMART program into how they practise.
• The person must be willing and able to take part for the full length of the study, including all phases.
• The person must be willing to share data about their prescribing habits and their asthma patients' outcomes throughout the study.

Patients:

• Patients who are aged 12 years or older may be considered.
• Patients with a formal diagnosis of asthma of any severity, recorded using the standard medical coding system, may be considered.
• Patients who have had 2 or more asthma attacks in the past year that required steroid treatment, and/or patients with an active prescription for certain types of asthma inhalers (medium or higher dose steroid inhalers, or low or higher dose combination steroid and long-acting reliever inhalers, along with a short-acting reliever inhaler) may be considered.

Who may not be able to join:

• Clinics that do not meet all four of the clinic-level requirements listed above may not be eligible to participate.
• Clinicians or staff who do not hold a current valid state licence in one of the listed roles may not be eligible to participate.
• Patients under the age of 12 may not be eligible to participate.
• Patients without a formally recorded asthma diagnosis, or who do not meet the prescribing or exacerbation history requirements, may not be eligible to participate.
