Who might be able to join this trial:

• People aged between 18 and 80 years old, of any gender
• People whose bowel wall thickness measures more than 3.0mm in the small intestine (ileum), as seen on an ultrasound scan; bowel disease in the colon is also permitted
• People who have never previously been treated with a medication called guselkumab
• People with a narrowing (stricture) in the bowel, where the bowel wall is thickened, the opening is less than 1cm or less than half the width of the nearby healthy bowel, and where the bowel just before the narrowing is stretched or dilated — both first-time narrowings and those at a surgical join site are permitted
• People with Crohn's disease in the small intestine who have no evidence of a narrowing on ultrasound or specialist bowel scans, or who have a type of Crohn's disease involving tunnel-like passages (fistulas)

Who may not be able to join:

• People who are pregnant
• People who have had surgical creation of an opening from the small intestine or large intestine through the abdomen (ileostomy or colostomy)
• People with significant obesity, defined as a BMI greater than 35
• People with a medical reason why starting the medication guselkumab would not be safe, such as having an active infection (confirm with trial site)
• People who have had an active cancer diagnosis within the past five years
• People who have a condition causing scarring (fibrosis) in other organs such as the lungs, kidneys, brain, or skin
