Who might be able to join this trial:
- You are 60 years of age or older
- You are currently being seen by a rheumatologist at the MUHC (McGill University Health Centre) for a joint or immune system condition — such as rheumatoid arthritis, psoriatic arthritis, lupus, scleroderma, gout, osteoarthritis, or a similar condition
- You are currently taking 5 or more regular medications, and at least one of those medications is considered potentially unsuitable for older adults (confirm with trial site)
- You are expected to continue having regular rheumatology check-up appointments roughly every 3 to 9 months
Who may not be able to join:
- You are not able to give your own informed consent to participate in the study
- You have a serious illness that is immediately life-threatening, or your doctors estimate you have less than 12 months to live
