Who might be able to join this trial:

• Adults aged 18 to 80 years (inclusive)
• People who have been diagnosed with an acute ischemic stroke (a stroke caused by a blocked blood vessel)
• People for whom doctors have planned to give clot-dissolving medication through a drip within 4.5 hours of stroke symptoms starting
• People whose blood pressure is high before receiving that clot-dissolving treatment — specifically, two consecutive readings within 2 minutes showing the top number at 180 or above, or the bottom number at 100 or above
• People who have given their own signed consent to participate, or whose legal representative has signed on their behalf

Who may not be able to join:

• People whose brain scan (CT or MRI) shows any bleeding in or around the brain, such as a brain bleed, bleeding around the brain, or blood pooling between the brain and skull
• People with a previous history of bleeding in the brain, or who have had a serious head injury or stroke in the last 3 months
• People with a tumour in the brain or a very large brain aneurysm (a dangerous bulge in a blood vessel)
• People who have had brain or spinal surgery in the last 3 months, major surgery in the last 2 weeks, or a procedure involving a needle in an artery in a hard-to-compress area within the last 7 days
• People who have had bleeding from the stomach, bowel, or urinary system in the last 3 weeks
• People with a serious condition where the main large artery from the heart has a tear in its wall (aortic dissection)
• People with a low platelet count (below 100×10⁹/L) or other conditions that make bleeding harder to control
• People who have taken certain blood-thinning medications recently — including low-molecular-weight heparin in the last 24 hours, warfarin with specific blood test results above certain thresholds, or newer anticoagulant medicines within the last 48 hours
• People whose blood sugar level is very low (below 2.8 mmol/L) or very high (above 22.2 mmol/L)
• People whose brain scan shows a large area of stroke damage affecting more than one-third of a major brain territory
• People with active internal bleeding, a known bleeding disorder, or a significant bleeding problem within the past 6 months
• People with a very severe stroke, as measured by a standard medical stroke severity scale (a score above 25 on the NIHSS) (confirm with trial site)
• People who had a seizure or fit at the time their stroke began
• People who are pregnant or breastfeeding
• People with a serious illness where life expectancy is 3 months or less
• People whose treating doctor considers that taking part in the study may be harmful to them
• People who are currently taking part in another clinical trial of a drug or medical device
• People who had significant disability before this stroke, as measured by a standard disability scale (an mRS score above 2 before the stroke occurred) (confirm with trial site)
