Who might be able to join this trial:
- Adults between 21 and 80 years of age
- People who are able to give their own consent to participate

Who may not be able to join:
- People with very severe kidney disease (stage 5) or who are currently on dialysis
- People who have been diagnosed with cancer within the past 3 years
- People who are already being planned for a procedure to open or bypass blocked arteries in the heart or neck
- People who have a history of inflammation of the pancreas (pancreatitis)
- People who have a history of a specific type of thyroid cancer called medullary thyroid cancer
- People who have a heart rhythm condition called atrial fibrillation
- People who have any other condition that the trial doctor believes would make it difficult to follow the trial requirements
- People with diabetes who are currently taking certain diabetes medications (called SGLT2 inhibitors or pioglitazone), unless those medications can be safely stopped — and people on another diabetes medication called a DPP-IV inhibitor who are placed in the treatment group would need to stop that medication during the trial (confirm with trial site regarding your specific medications)
- People who have a known allergy to a medication called Acetazolamide
- Women who are pregnant or breastfeeding

Primary endpoints
To evaluate whether GLP1 RA (Dulaglutide) therapy would improveme cerebral vasodilatory reserve by at least 4 points in patients with severe stenosis of ICA or MCA
