Who might be able to join this trial:

• Adults aged 18 or older, of any gender
• People with unresectable, recurrent, or advanced spreading esophageal squamous cell carcinoma (a specific type of esophageal cancer) confirmed by laboratory tissue testing — certain mixed or other cancer types are not included
• People who previously received chemotherapy or radiation therapy before or after surgery, or combined chemotherapy and radiation as a main treatment, where the cancer came back more than 6 months after that treatment ended
• People with a physical fitness score of ECOG 0 or 1, meaning they are fully active or have minor restrictions on physically demanding activity (confirm with trial site)
• People who have at least one measurable area of cancer, as assessed by standard imaging guidelines called RECIST v1.1
• People who are able to provide either a new or previously stored tissue sample for laboratory testing of a protein called PD-L1
• People whose blood, heart, lung, liver, and kidney functions are within normal or acceptable ranges, with no immune deficiency diseases
• People with normal blood clotting function and no active bleeding or blood clot conditions
• People with an expected survival of at least 12 weeks
• Women of childbearing age, and men whose partners are women of childbearing age, who are willing to use effective contraception throughout the treatment period and for 6 months after treatment ends
• People who are willing to sign a written consent form and are able to follow the trial visit schedule and procedures

Who may not be able to join:

• People whose esophageal cancer could potentially be cured by surgery or radiation therapy alone
• People with esophageal squamous cell carcinoma that is completely blocking the esophagus and requires a procedure to clear the blockage
• People where a stent placed in the esophagus or airway would carry a risk of causing a hole (perforation) in that tissue
• People who have previously received systemic (whole-body) treatment for advanced or metastatic esophageal squamous cell carcinoma
• People diagnosed with another type of cancer within the past 5 years — exceptions may apply for certain skin cancers or cervical or breast cancers that were fully treated (confirm with trial site)
• People with a serious active infection, or active lung inflammation
• People with a current or past history of a lung scarring condition, dust-related lung disease, drug-related lung inflammation, or severely reduced lung function
• People with an active tuberculosis infection
• People currently participating in another treatment-based clinical trial — being in a follow-up phase or an observational study may be acceptable (confirm with trial site)
• People with conditions that weaken the immune system, including HIV infection, active hepatitis B, active hepatitis C, or a combined hepatitis B and C infection
• People who have had an organ transplant or bone marrow/stem cell transplant from another person
• People who have had major surgery (such as brain, chest, or abdominal surgery) within 4 weeks before starting the trial, or who expect to have major surgery during the trial period
• People with active symptoms from cancer spread to the brain or the lining of the brain and spinal cord — stable brain spread that does not need steroid treatment may be an exception (confirm with trial site)
• People with an active autoimmune disease requiring treatment, or a history of such a disease in the past 2 years — certain conditions such as vitiligo, non-treated psoriasis or hair loss, controlled thyroid conditions, or type 1 diabetes managed with insulin alone may be exceptions (confirm with trial site)
• People who are pregnant or breastfeeding
• People with any other serious or uncontrolled health condition that the research team considers would increase the risk of participating
