Who might be able to join this trial:
- People aged 18 or older
- People who have had an acute stroke caused by a blocked blood vessel, treated with a standard dose of clot-dissolving medication (given through a vein) within 4.5 hours of symptoms starting, and for whom no surgical clot-removal procedure is planned
- People who had a measurable level of neurological impairment before the first clot-dissolving treatment, based on a standard stroke severity score (NIHSS score of 4 or higher)
- People whose brain scans show a certain minimum level of healthy brain tissue remaining (specific scoring thresholds apply — confirm with trial site)
- People who showed little or no improvement, or who got worse after initially improving, within 1 hour of receiving the first clot-dissolving treatment

- People for whom a second dose of clot-dissolving medication can be given within 6 hours of stroke symptoms starting

- People who have signed a consent form agreeing to take part
Who may not be able to join:
- People who are planned to undergo a procedure to physically remove the clot from inside a blood vessel
- People with significant changes to the white matter of the brain shown on imaging (a specific severity level applies — confirm with trial site)
- People who had abnormal blood clotting results before the first treatment, including certain elevated clotting measures
- People who took two different blood-thinning/antiplatelet medications within 24 hours before the clot-dissolving treatment
- People who are pregnant
- People who have a known allergy to the medication(s) being studied
- People who have other serious health conditions alongside the stroke
- People who have taken part in another clinical trial within the past 3 months
- People the research team considers unsuitable for the study for other reasons
