Who might be able to join this trial:

• Men aged 18 years or older
• People who have been diagnosed with prostate cancer (a specific type called adenocarcinoma) confirmed by a tissue sample, where the treating doctor has recommended radiation treatment to the whole prostate
• People whose cancer has a Gleason score of 3+3, 3+4, or 4+3 (also called Grade Group 1, 2, or 3), which generally indicates low to intermediate risk disease

• People whose cancer appears to be contained within or only very minimally outside the prostate gland on MRI, with favourable intermediate-risk features (confirm with trial site)
• People whose PSA level was below 20 ng/ml before starting hormone therapy
• People with a general health and activity level rated as WHO Performance Status 0–2, meaning they are able to carry out daily activities with no more than moderate limitation
• People who are willing and able to sign a consent form agreeing to participate
• People who are willing and able to complete health and wellbeing questionnaires at regular points throughout the study
• People whose MRI scan was performed within 3 months of entering the trial

Who may not be able to join:

• People for whom an MRI scan is not safe, such as those with a pacemaker, certain metal implants in the body, or severe claustrophobia
• People with a urinary symptom score (IPSS) above 19, indicating significant urinary problems
• People whose higher-grade cancer (Grade Group 3) is found outside the area visible on MRI, beyond what a treating doctor would consider suitable for monitoring rather than active treatment (confirm with trial site)
• People with a prostate gland larger than 90cc in volume
• People with other health conditions that could increase the risk of serious side effects from treatment, such as inflammatory bowel disease, or conditions that would prevent long-term follow-up
• People with a hip replacement or other metal implants in the pelvic area that interfere with MRI scan quality
• People who have had radiotherapy to the pelvic area in the past
• People whose disease requires more than 6 months of hormone therapy
• People who have had another type of cancer within the past 2 years — though certain exceptions may apply, including some skin cancers, certain low-risk bladder cancers, and small kidney growths being monitored (confirm with trial site)
