Who might be able to join this trial:
- People aged 18 years or older
- People whose general health and ability to carry out daily activities meets a standard medical performance rating of 0 or 1 (meaning fully active or able to carry out light activity) — confirm with trial site if unsure

- People whose prior scans (CT, bone scan, MRI, or PSMA PET) have shown cancer that has spread to lymph nodes or other parts of the body
- People for whom doctors are planning to add stronger hormone-blocking medications or chemotherapy to their treatment
- People whose cancer has been assessed as T4 stage, meaning it has grown into nearby structures (confirm with trial site)
- People who have previously received hormone therapy (ADT) for prostate cancer
- People who have previously had radiation treatment to the pelvis
- People who have previously had prostate surgery, including a procedure called TURP (a surgery to relieve urinary blockage)
- People with a urinary symptom score greater than 20 on a standard questionnaire (indicating significant urinary problems)
- People who have previously had a narrowing of the urethra (the tube that carries urine)
- People whose prostate measures larger than 80 cc on MRI
- People with hip implants that interfere with imaging of the prostate area
- People who are unable to temporarily stop blood-thinning medication for a minor procedure to place small internal markers used in radiation planning
- People with active Crohn's disease or scleroderma
