Who might be able to join this trial:

• You are between 40 and 75 years old.
• You have been officially diagnosed with moderate to severe chronic obstructive pulmonary disease (COPD), a long-term lung condition that makes breathing difficult.
• Your condition has been stable for at least 4 weeks since your last serious flare-up, as confirmed by your lung specialist.
• Your doctor has referred you to an outpatient lung rehabilitation program because of ongoing breathing difficulties and a history of flare-ups, and your breathlessness has been rated as moderate to severe on a standard scale used by doctors.
• You have already completed at least 6 out of 8 sessions of a standard 8-week lung rehabilitation program.

Who may not be able to join:

• You have a medical condition that makes physical exercise unsafe, such as a recent heart attack, an uncontrolled heart rhythm problem, a severe muscle or nerve disorder, recent surgery, or a recent stroke (confirm with trial site).
• You have severe memory or thinking difficulties that would make it hard to follow instructions or take part in the program.
• You have an unstable mental health condition that might make it difficult to stick to a regular exercise schedule.
• You are unable to attend the scheduled sessions or do not wish to participate in the community-based exercise program.
• You have taken part in another lung rehabilitation program in the 6 months before joining this study.
