Who might be able to join this trial:
- People who have been diagnosed with prostate cancer confirmed by a tissue biopsy, specifically the type called adenocarcinoma.
- People whose prostate cancer has spread to nearby lymph nodes in the pelvis, but not to other parts of the body.
- People whose affected lymph nodes are only in specific pelvic regions (the obturator, internal iliac, external iliac, or presacral areas), as shown on imaging scans.
- People with no signs of cancer spread to distant lymph nodes, bones, or other organs.
- People for whom radiation therapy to the prostate and nearby pelvic lymph nodes is considered an appropriate treatment option by their medical team.

- People who are reasonably active and able to carry out daily activities — rated 0, 1, or 2 on a standard medical fitness scale (confirm with trial site).
- People whose kidneys, liver, and other organs are functioning well enough to safely receive the planned treatments.
- People aged 18 or older.
- People who are able to understand the study and are willing to sign a consent form agreeing to take part.
Who may not be able to join:
- People whose cancer has already spread to distant parts of the body, such as bones, organs, or lymph nodes outside the pelvis.
- People who have already had a major treatment for their prostate cancer, such as surgical removal of the prostate, full-course radiation to the prostate, or internal radiation (brachytherapy).
- People who have previously had radiation therapy to the pelvic area for any reason, if that treatment would overlap with the planned radiation in this trial.
- People who have already received systemic (whole-body) drug treatment for prostate cancer, other than short-term hormone therapy specifically permitted by the study.
- People who have a history of prostate cancer that stopped responding to hormone treatment (known as castration-resistant prostate cancer).
- People who currently have another cancer that needs active treatment — though some exceptions may apply for certain low-risk skin cancers or other cancers unlikely to affect the study (confirm with trial site).
- People with serious uncontrolled health conditions that could make it unsafe to receive radiation therapy or the other treatments in the study.
- People with any other condition that the treating doctor believes could affect safety or the ability to follow the study requirements.
