Who might be able to join this trial:
- You have had a stroke (either a blocked blood vessel or a bleed in the brain) and needed a tracheostomy (a breathing tube placed in your neck) as a result.
- Your stroke is in a stable, recovery phase — meaning your condition is no longer in crisis, brain swelling has gone down, and no new stroke symptoms have appeared.
- You have been breathing without a ventilator (breathing machine) for at least 48 hours before joining the trial.
- Your vital signs are stable: normal temperature (below 38°C/100.4°F), heart rate between 60–100 beats per minute, breathing rate under 20 breaths per minute, and blood pressure within a safe range.
- You do not have a serious active infection that requires antibiotics given through a drip (IV).
- Your organs are functioning properly and your carbon dioxide levels in the blood are within a safe range.
- Your cough is strong enough to clear your airways (measured by a specific cough strength test) (confirm with trial site).
- The amount of secretions (mucus/fluid) collected from around your airway over 24 hours is small (less than 20 ml).
- You or your legal representative is willing and able to sign a consent form to take part.
Who may not be able to join:
- You do not tolerate having the air cuff on your tracheostomy tube deflated — for example, if it causes severe coughing, difficulty breathing, low oxygen levels, or fast breathing lasting more than 5 minutes.
- You have a serious structural problem with your airway, such as a significant narrowing of the windpipe (more than half blocked) or an abnormal connection between the windpipe and the food pipe.
- Your life expectancy is less than 2 weeks, or you are expected to be moved to a hospital or facility not involved in this trial within 2 weeks.
- You have a severe, uncontrolled illness such as advanced cancer or serious heart failure.
