Who might be able to join this trial:

• Adults between the ages of 40 and 75 years.
• People who have been diagnosed with a specific severity of Chronic Obstructive Pulmonary Disease (COPD), classified as "Group E" under international guidelines from 2023.
• People who have been using a combination of three inhaled medications for COPD (one from each of these types: a long-acting airway-relaxing medicine, an inhaled steroid, and another type of long-acting airway-relaxing medicine) for at least 6 months before joining.
• People whose COPD has been stable, with no significant changes or flare-ups, for at least 2 weeks before joining.
• People who have agreed in writing to take part in the study.

Who may not be able to join:

• People who currently smoke, or who stopped smoking less than 6 months before the screening appointment.
• People who have had a COPD flare-up (a sudden worsening of symptoms) within the 2 weeks before joining.
• People who have been diagnosed with another lung condition in addition to COPD, such as tuberculosis, blood clots in the lungs, a collapsed lung, large air pockets in the lungs, asthma, scarring of the lungs, or lung cancer.
• People who have had tuberculosis at any point in the past 10 years.
• People who have an active infection of any kind, including HIV.
• People who have been diagnosed with any type of cancer.
• People with serious heart conditions, such as severe heart failure, significant irregular heartbeat, heart valve problems, heart muscle disease, or a heart condition present from birth.
• People whose liver is not functioning well, based on blood test results showing certain liver-related levels more than twice the normal upper limit.
• People whose kidneys are not functioning well, based on a blood test result for creatinine showing more than one and a half times the normal upper limit.
• People who are pregnant or breastfeeding.
• People who have other serious health conditions that may affect how long they live, such as poorly controlled diabetes, a recent heart attack, unstable chest pain, severe liver scarring, or acute kidney inflammation.
• People whose blood tests show a low white blood cell count, which may indicate a weakened immune system.
• People who have a history of a psychiatric illness, epilepsy, or other conditions affecting the brain or nervous system.
• People who have a history of alcohol or drug misuse.
• People who have taken part in another clinical trial within the 3 months before joining.
• People who have had difficulty following medical care in the past, or who are expected to find it hard to complete all parts of the study.
• People who are unable to perform a breathing test called spirometry, which requires following specific breathing instructions.
• People who, due to other health conditions, are not expected to live for more than 6 months.
• People who have used medications that suppress the immune system within the 8 months before the screening appointment.
