Who might be able to join this trial:

• You are between 18 and 80 years old and have been diagnosed with moderate-to-severe Crohn's disease.
• You are currently being treated with one of two specific medications: Guselkumab or Ustekinumab.
• In the past 8 weeks, you have had symptoms suggesting a partial or full blockage in your intestine, including stomach pain after eating that is linked to a narrowing in your bowel (confirm with trial site for exact symptom details).
• Scans or camera tests (such as a CT scan or endoscopy) have confirmed that there is a real narrowing in your intestine caused by your Crohn's disease, meeting specific measurable criteria on imaging or being too narrow for a camera to pass through.

Who may not be able to join:

• You need emergency surgery or an urgent procedure, or your doctor believes you will need to change your medication or have planned surgery within the next 2 months.
• Your bowel narrowing or blockage is caused by a previous surgery, rather than by Crohn's disease itself.
• You have had a procedure to treat a bowel narrowing in the past 6 months (such as balloon stretching, cutting, or surgical widening of the bowel).
• You have been unable to eat or drink by mouth for more than 2 months, even with tube feeding support.
• You have used Guselkumab, Ustekinumab, or similar medications (IL-23 blockers) in the past 12 months, or you have a known allergy or medical reason preventing you from taking these medications.
• You are unable to have a CT scan, for example due to an allergy to the contrast dye used in the scan.
• You have certain health conditions that make it unsafe to use biological medicines, such as active tuberculosis, or a history of heart attack, heart failure, or certain nerve conditions in the past 5 years.
• You currently have or have previously had certain cancers, including solid tumours, lymphoma, or melanoma, or you are currently receiving chemotherapy or radiotherapy.
• You have certain intestinal conditions such as short bowel syndrome, a stoma (colostomy), or growths in the colon or rectum.
• You have serious active health problems such as major intestinal bleeding, severe liver or kidney problems, active infection, shock, uncontrollable vomiting, or severe difficulty absorbing nutrients.
• You are pregnant or breastfeeding.
• Your overall health condition is very unstable, rapidly worsening, or considered end-stage.
• You have a psychiatric disorder, or there are concerns about your ability to fully understand and participate in the study (confirm with trial site).
