Who might be able to join this trial:
- You have been diagnosed with rheumatic valvular heart disease (a heart valve problem caused by rheumatic disease)
- You are between 18 and 80 years old
- You have been told you have severe pulmonary hypertension (very high blood pressure in the arteries going to your lungs)
Who may not be able to join:
- You are scheduled to have corrective heart surgery within the next 6 weeks

- Your blood pressure is too low (below 90/50 mmHg)
- You have heart failure or a heart artery condition with unstable chest pain (confirm with trial site)
- You have had an allergic reaction or sensitivity to sildenafil or any of its ingredients
- You have previously had a specific type of eye condition affecting the optic nerve called non-arteritic anterior ischemic neuropathy (confirm with trial site)
- You have had a stroke in the last 6 months
- You have a serious, life-threatening irregular heartbeat
- You have had a heart attack in the last 6 months
- You currently take nitrate medications (such as certain chest pain drugs) as a necessary part of your treatment
- You do not have any plans for corrective heart surgery within the next 6 weeks
