Who might be able to join this trial:

• You are 18 years old or older, of any sex
• You have had an acute stroke caused by a blocked basilar artery (a major artery at the base of the brain)
• You and your care team have decided to go ahead with an emergency procedure to open the blocked artery (endovascular treatment)
• The time between your stroke starting and the start of the procedure is within 24 hours
• Your stroke symptoms are rated as moderately severe or severe based on a standard medical stroke scoring tool (score of 10 or higher on the NIHSS)
• A brain scan shows that the damage to the back part of your brain is not too extensive (score of 6 or higher on a specific CT scan scoring system)
• You, or someone legally authorised to act on your behalf, have signed a consent form to participate

Who may not be able to join:

• You have any type of bleeding in or around the brain (such as a brain bleed or bleeding around the brain's surface)
• You had significant disability before this stroke (confirm with trial site)
• You have a known allergy to the study drug tocilizumab or any of its ingredients
• You have a known allergy to the dye used in imaging scans (iodinated contrast agents)
• Your blood vessels are too twisted or curved to safely complete the artery-opening procedure (confirm with trial site)
• You have a history of bleeding disorders, problems with blood clotting, or conditions involving low platelet counts
• Your blood pressure remains very high (systolic 180 mmHg or above, or diastolic 110 mmHg or above) even after attempts to lower it
• Your white blood cell count (specifically neutrophils) is too low, based on a blood test
• Your platelet count (cells that help blood clot) is too low, based on a blood test
• Your blood sugar level is either too low or too high at the time of the trial
• Your liver enzyme levels (ALT or AST) are more than twice the normal upper limit, based on a blood test
• Your kidney function is significantly reduced, based on blood tests
• You are pregnant, breastfeeding, or planning to become pregnant within 90 days
• You have a severe mental health condition or dementia that would prevent you from giving consent or attending follow-up appointments
• You have an active cancer or a serious illness where life expectancy is less than 90 days
• You have an autoimmune disease or are currently taking medicines that suppress the immune system
• You have an active infection affecting your whole body
• You have taken part in another interventional clinical trial in the last 30 days, or are currently in one
• The trial doctors have any other concern that joining the trial could be harmful to you
• The trial doctors feel there is any other reason that might prevent you from fully participating or following the study requirements (confirm with trial site)
