Who might be able to join this trial:

• People aged between 18 and 80 years old
• People who have had a blockage in a major blood vessel supplying the front part of the brain, confirmed by a brain scan
• People whose stroke is severe enough to score 6 or higher on a standard stroke severity assessment scale (called the NIHSS)
• People whose brain scan shows that a large enough area of brain tissue is still potentially salvageable, based on a scoring system called ASPECTS (score of 6 or higher)
• People who were largely independent before their stroke, with little to no disability beforehand
• People for whom doctors have planned a procedure to physically remove or clear the blood clot (called endovascular treatment) within 24 hours
• People, or their legal representative, who are able to provide written agreement to take part

Who may not be able to join:

• People who have bleeding in or around the brain shown on a head scan
• People with a history of bleeding disorders, problems with blood clotting, or low platelet counts
• People whose blood vessel shape or structure is likely to make the clot-removal procedure technically too difficult to perform
• People with a known allergy to contrast dye used in medical imaging
• People who are pregnant, breastfeeding, or planning to become pregnant during the study or within 90 days of joining
• People with a known allergy or severe reaction to a medication called anisodine hydrobromide
• People with certain medical conditions that could be made worse by a type of medication called anticholinergics — including a specific type of eye pressure condition (angle-closure glaucoma), difficulty passing urine, or a serious bowel condition (confirm with trial site)
• People with serious heart rhythm problems or unstable blood pressure that requires urgent medical support
• People with a severe psychiatric illness, significant memory or thinking problems, or reduced consciousness that would prevent them from giving consent or completing follow-up
• People with a terminal cancer diagnosis or another serious illness where survival of less than 90 days is expected
• People who are currently in another interventional clinical trial, or who have been in one within the past 30 days
• People who have any other condition that the trial doctor considers would make participation unsuitable
