Who might be able to join this trial:
- Adults aged 18 or older
- People who have had an acute stroke caused by a blocked blood vessel, treated with an intravenous clot-dissolving drug called prourokinase within 4.5 hours of symptoms starting, and for whom surgery to remove the clot is not planned
- People who had measurable stroke symptoms before receiving the clot-dissolving treatment, with a stroke severity score (NIHSS) of 4 or higher
- People whose brain scan at the start shows a sufficient amount of healthy brain tissue remaining (specific scan scores will be assessed by the trial site)
- People who showed little or no improvement — or who got worse after initial improvement — at one hour after receiving the first clot-dissolving treatment
- People whose follow-up scan at one hour after the first treatment confirms a blocked blood vessel is still present and rules out bleeding in the brain
- People for whom a second dose of the clot-dissolving drug can be given within 6 hours of stroke symptoms starting
- People experiencing their first stroke, or who have had a previous stroke but recovered well with little or no lasting disability
- People who have given signed consent to take part
Who may not be able to join:
- People for whom a procedure to physically remove the clot from inside a blood vessel is planned
- People with significant age-related changes to the white matter of the brain (confirm with trial site)
- People with blood clotting problems before the first treatment, including certain abnormal blood test results (such as INR above 1.5)
- People who took two different blood-thinning medications together in the 24 hours before treatment
- People who are pregnant
- People with a known allergy to the study drug or drugs
- People with other serious medical conditions
- People who have participated in another clinical trial within the past 3 months
- People the research team considers unsuitable for the study for other reasons (confirm with trial site)
