Who might be able to join this trial:

• You are showing signs of a stroke affecting the front part of the brain (anterior circulation)
• A scan (CTA, MRA, or DSA) has confirmed a blockage in a major blood vessel in the front part of the brain (such as the internal carotid artery or middle cerebral artery)
• You are having a procedure to remove the blood clot (mechanical thrombectomy)
• You are between 18 and 80 years old (male or female)
• Before this stroke, you were largely independent in daily life (little or no disability)
• The time between when you were last known to be well and the clot-removal procedure is 24 hours or less — this includes strokes noticed upon waking up
• Your stroke severity score on a standard medical scale (NIHSS) is 6 or higher when you arrive at the hospital
• A brain scan score (ASPECTS) shows a result of 3 or higher, indicating some healthy brain tissue remains
• You or your legal representative are able to give written consent to participate

Who may not be able to join:

• You have blockages in both the front and back of the brain, or blockages on both sides of the front of the brain at the same time
• A standard stroke severity score (NIHSS) could not be measured before you were sedated or put on a breathing machine
• You had a seizure at the start of your stroke that prevented doctors from measuring your stroke severity score
• Both of your pupils are enlarged and unresponsive (confirm with trial site)
• You have a known allergy to the study drug (sivelestat sodium) or any of its ingredients
• You have a severe allergy or medical reason that means you cannot receive the contrast dye used in scans
• Your blood pressure is very high and cannot be brought down with medication (above 185/110 mmHg)
• Your blood sugar level is either very low (below 50 mg/dL) or very high (above 400 mg/dL)
• Your platelet count (cells that help blood clot) is too low
• You have a bleeding disorder, a clotting factor deficiency, or you have been taking blood-thinning medication recently with certain lab values outside the acceptable range
• You have severe kidney disease or are on dialysis
• You are unlikely to be able to complete a 90-day follow-up (for example, if you have no fixed address or live overseas)
• Your stroke is thought to be caused by blood vessel inflammation (vasculitis) or an infected clot
• There is a suspicion that your symptoms may be caused by a tear in the main artery from your heart (aortic dissection)
• You have a brain tumour (except a small, slow-growing type called a meningioma), active bleeding in the brain, or an abnormal tangle of blood vessels in the brain
• Brain imaging shows significant swelling causing a shift in brain structures
• There is evidence of a tear in the carotid artery that is restricting blood flow
• You have a pre-existing neurological or psychiatric condition that could affect how doctors assess your condition
• You are pregnant or breastfeeding
• You have a diagnosed rheumatic or autoimmune disease and are taking long-term immune-suppressing or steroid medications
• You are currently receiving chemotherapy or certain other medications that affect the immune system (confirm specific medications with trial site)
• You are already taking part in another clinical trial that could affect the results of this study
• The trial doctors believe, for any other reason, that joining the trial would not be safe or suitable for you
