Who might be able to join this trial:

  • You must be 18 years old or older
  • You must be getting ready to leave an inpatient rehabilitation program at the Bern Rehab Center in Heiligenschwendi or the Insel Hospital, with no more than 10 days left before discharge
  • You must belong to one of these groups: people who have had a heart attack or heart failure caused by blocked arteries; people diagnosed with COPD (a long-term lung condition) confirmed by a lung specialist; people who have had a bone fracture caused by a minor fall or injury (such as a hip fracture); or people who have had a mild stroke (either a blockage or bleeding in the brain) with only mild lasting effects
  • You must be able to understand and agree to take part in the study on your own
  • You must have access to a device that connects to the internet, such as a smartphone, tablet, or laptop, and be able to use it

Who may not be able to join:

  • People who have a mental health condition or memory/thinking difficulties that would make it hard to give consent or use an app
  • People who have a physical disability that means they cannot use a digital device on their own without help
  • People who have significant difficulty speaking, understanding, reading, or processing language
