Who might be able to join this trial:

• Men aged between 18 and 79, from any ethnic background, who have been diagnosed with a specific stage of prostate cancer (classified as cT2–T3a, with no spread to nearby lymph nodes or other parts of the body)
• People who had a specialised prostate MRI scan (called an mpMRI) done before their prostate biopsy
• People who are healthy enough to undergo a type of prostate removal surgery performed with robotic assistance (called RARP)
• People whose biopsy results and MRI scan are of good enough quality to help plan the surgery
• People who are already scheduled for this type of robotic prostate surgery, where the surgical team has recommended preserving the nerve bundles involved in erections, based on team discussions and scan and biopsy results
• Men who are sexually active and have no or only mild erection difficulties before the trial, based on a specific questionnaire score (IIEF-EFD score of 24 or higher)
• People who want to preserve their ability to have erections for sexual intercourse
• People who are able to read English well enough to understand the study information and give their informed agreement to take part

Who may not be able to join:

• People who already have moderate or severe erection difficulties (IIEF-EFD score below 24)
• People who received hormone-reducing therapy (androgen deprivation therapy) before surgery
• People who have previously had surgery for a non-cancerous enlarged prostate
• People who have already received any treatment for prostate cancer, including external radiotherapy, internal radiation (brachytherapy), targeted local treatment (focal therapy), or chemotherapy
• People who have had a previous pelvic injury involving a fracture, or a penile fracture
• People who have previously had surgery to treat erection difficulties
• People whose prostate MRI scan was of poor quality, or who only had a type of MRI scan done without contrast dye
• People who have an established blood vessel or circulation condition, such as heart disease, a history of stroke, or problems with blood flow in the legs or other areas (confirm with trial site)
