Who might be able to join this trial:

• People who have been diagnosed with a single, new liver cancer (hepatocellular carcinoma) measuring 8 cm or smaller, confirmed by tissue sample or imaging scan, at an early stage (BCLC stage 0 or A).
• People with a previous liver cancer diagnosis may still be considered, provided that earlier cancer was a single tumour of 5 cm or smaller, was located in a different part of the liver than the current cancer, and was treated with curative intent more than 2 years ago with no signs of the cancer returning at that site.
• People whose liver cancer specialist team has agreed they are suitable for ablation (heat-based tumour destruction) and/or artery-based treatments, are not suitable for or have declined surgery to remove part of the liver, and are not being planned for a liver transplant.
• People whose liver function falls within a certain acceptable range, measured by a score called Child-Pugh (score of B7 or better), with no fluid build-up in the abdomen, or fluid that is controlled with water tablets.
• People with a general health and activity level rated at ECOG 2 or better (meaning they are able to carry out some daily activities, even if limited).
• People whose blood test results meet minimum levels for platelets, haemoglobin, neutrophils, and a clotting measure called INR (confirm with trial site for exact values, as exceptions may apply for people on blood-thinning medication).
• People aged 18 years or older who are able to provide written consent to participate.

Who may not be able to join:

• People whose liver cancer has spread to multiple spots in the liver, has grown into major blood vessels, or has spread outside the liver.
• People who have received any treatment for liver cancer within the past 2 years.
• People who have noticeable fluid build-up in the abdomen (ascites) or a condition called hepatic encephalopathy, where the liver is not filtering toxins properly and this is affecting brain function.
• People who have had previous radiation treatment to the abdomen in a way that would prevent the specific radiation therapy used in this trial from being safely delivered to the tumour.
• People with untreated Hepatitis B or Hepatitis C infection.
• People who have another active cancer (other than certain common skin cancers) that has been growing or required treatment within the last 2 years.
• People who are pregnant.
