Who might be able to join this trial:

• People who are willing to take part voluntarily and sign a consent form
• People aged between 18 and 75 years old
• People who have been confirmed by biopsy to have squamous cell carcinoma (a specific type of cancer) in the chest portion of the food pipe (esophagus), with no spread to distant parts of the body, and whose cancer has been assessed by scans, specialist imaging, and a team of doctors as being on the borderline of whether surgery is possible
• People who have not previously received chemotherapy or radiotherapy
• People whose doctors estimate they are likely to live for at least 3 more months
• People who are generally well enough to carry out light activity and care for themselves (assessed using a standard medical scale scored 0 or 1)
• People whose recent blood tests (within the past 7 days) show healthy enough levels of red blood cells, infection-fighting white blood cells, and platelets
• People whose recent blood tests (within the past 7 days) show that the liver and kidneys are working within acceptable ranges
• People who do not have serious complications such as major internal bleeding, a hole in the digestive tract, severe jaundice, bowel blockage, or a fever above 38°C that is not caused by the cancer
• People of childbearing potential who are willing to use effective contraception throughout the study
• People who are able to attend all required follow-up appointments and comply with the study plan

Who may not be able to join:

• People whose cancer is located in the neck portion of the food pipe (rather than the chest portion)
• People whose cancer has spread to distant parts of the body
• People whose food pipe is almost completely blocked, confirmed by internal camera examination, requiring a procedure to relieve the blockage
• People who have previously had a stent (a small tube) placed in their food pipe or airway
• People whose tumour has visibly grown into nearby major blood vessels or the airway, creating a high risk of bleeding or a hole forming, or who already have an abnormal connection (fistula) between the food pipe and another organ
• People who currently have, or have had, another separate cancer — except for certain skin cancers or pre-cancerous cervical cell changes that have been fully treated and cured
• People who have taken immune-suppressing medications within the week before enrolling — though certain low-dose or topical steroid treatments may be acceptable (confirm with trial site)
• People who have an active autoimmune disease needing treatment, or who have had such a condition within the past 2 years
• People with a known history of a condition where the immune system does not work properly from birth or early life
• People with currently active tuberculosis
• People who have previously received a transplanted organ or bone marrow/stem cells from another person
• People with a lung condition called interstitial lung disease that requires steroid treatment
• People who are known to be allergic to any monoclonal antibody medicines, or to chemotherapy drugs in the taxane or carboplatin families, or to the inactive ingredients in these medicines
• People with serious heart conditions, including heart failure, uncontrolled abnormal heart rhythms, medically managed chest pain, significant heart valve problems, a history of severe heart attack, or very difficult-to-control high blood pressure
• People with ongoing frequent loose or watery stools (4 or more per day) or poor kidney function
• People with an active infection or an active contagious illness
• People with neurological or mental health conditions that affect thinking or understanding
• People who are pregnant or breastfeeding
• People with other significant health conditions, mental health disorders, or abnormal test results that the trial doctors consider would make participation unsafe or affect the reliability of the results
