Who might be able to join this trial:

• People aged 18 or older
• People who have been diagnosed with severe or very severe COPD (classified as GOLD stage 3 or 4)
• People whose lungs show signs of being over-inflated (trapping too much air), confirmed by a specific breathing test called a nitrogen wash-out test, with at least one abnormal result from that test
• People whose condition has been stable, meaning no flare-ups (worsening of symptoms) in the 3 weeks before joining the trial
• People whose treating doctor considers it clinically appropriate to explore whether the Simeox® device may help reduce lung over-inflation
• People who are willing and able to give written consent to participate

Who may not be able to join:

• People who have had a collapsed lung (pneumothorax) recently (within the last 6 weeks), or who have a known tendency toward this condition or known fragility of the lining around the lungs
• People who have had significant coughing up of blood or active bleeding in the lungs recently (within the last 6 weeks)
• People whose blood pressure or circulation is currently unstable
• People with certain unstable heart conditions, including a recent heart attack, unstable chest pain (angina), uncontrolled irregular heartbeat, or unstable heart failure
• People who have had surgery on the chest or food pipe (oesophagus) within the last 3 months
• People who have recently experienced a serious chest injury or severe sudden lung damage
• People who require continuous mechanical breathing support, active assistance with breathing in, or who have a breathing tube surgically placed in their throat (tracheostomy)
• People with nerve or muscle diseases that cause weakness in the muscles used for breathing
• People whose breathing-in muscles are too weak to handle increased breathing effort
• People with weakness in the muscles of the mouth or throat
• People who are unable to cough strongly and independently on their own
• People with severe stiffening or restriction of the lungs (where total lung capacity measures below 60% of what is expected for that person)
• People whose airways are at risk of inhaling fluids, for example those recently fed through a tube passed through the nose into the stomach
• People with gastro-oesophageal reflux disease (where stomach acid flows back into the food pipe) that is not well controlled
