Who might be able to join this trial:
- People who have had ongoing back pain for 3 months or longer, without a clear confirmed cause or diagnosis
- People whose back pain first started at age 45 or younger
- People who have been referred to a rheumatologist (a joint and inflammation specialist) to investigate a possible condition called axial spondyloarthritis or a similar condition, but who have not yet received a confirmed diagnosis

Who may not be able to join:
- People who have difficulty communicating due to cognitive difficulties, vision problems, or other factors that affect their ability to complete self-assessments
- People who already have a confirmed diagnosis of axial spondyloarthritis, ankylosing spondylitis, rheumatoid arthritis, psoriatic arthritis, or another established rheumatic disease
- People whose back pain has lasted less than 3 months, or whose back pain first began after the age of 45
- People who are unable to complete the full set of required tests — for example, those with metal implants that prevent an MRI scan, or those with serious liver or kidney conditions that prevent certain blood tests
- People who have a clearly identified and diagnosed cause of their back pain that is not related to spondyloarthritis, such as a slipped disc, spinal narrowing, a spinal tumour, a spinal infection, or a fracture related to bone thinning
- People who are unwilling or unable to give consent, attend follow-up appointments, or provide the information required throughout the study
