  • People who come to the emergency room with atrial fibrillation or atrial flutter that started less than 48 hours ago

  • People who have another serious illness happening at the same time, such as severe heart failure or a heart attack
  • People for whom blood thinning medication is not considered safe
  • People who have a mechanical heart valve or a specific type of heart valve narrowing called mitral stenosis
  • People who are expected to need to stay in hospital for more than 24 hours for any reason
  • People considered to be at an especially high risk of blood clots, for example those who have previously had a stroke caused by a blood clot despite being on blood thinners
  • People for whom a specific type of heart ultrasound performed through the throat (transesophageal echocardiography) is not suitable
