Who might be able to join this trial:
Colorectal cancer (CRC) group:
- People who have been diagnosed with colorectal cancer that has spread to the liver (confirmed by tissue or cell testing), classified as stage IV
- People for whom surgery and/or chemotherapy has been recommended as a treatment option
- People aged 18 or older
- People who are largely able to carry out normal daily activities with little or no limitation (as assessed by a standard activity scale called ECOG PS 0-1)
- People who are able and willing to sign a written consent form
- People who are being treated at IRCCS San Raffaele hospital
Pancreatic cancer (PDAC) group:
- People with a diagnosis or strong suspicion of pancreatic cancer that has spread to the liver (stage IV), confirmed or to be confirmed by tissue or cell testing
- People aged 18 or older
- People who are able to carry out at least some self-care and are not completely disabled (as assessed by a standard activity scale called Karnofsky, score of 50 or above)
- People whose liver tumour tissue samples were collected at IRCCS San Raffaele and stored in the hospital's biological sample bank
- People who are able and willing to sign a written consent form
- People with pancreatic cancer that has not yet spread, who are scheduled for pancreatic surgery at IRCCS San Raffaele
Who may not be able to join:
Colorectal cancer (CRC) group:
- People who are pregnant or breastfeeding
- People who are unable to sign a written consent form
- People whose cancer has spread to areas outside the liver, unless those areas are under control or in remission due to chemotherapy and surgery is still considered possible (confirm with trial site)
- People with serious other health conditions, such as heart disease, a history of psychiatric illness, HIV, or autoimmune disorders
- People who have had another cancer within the past 5 years, unless it was a fully treated minor skin cancer, superficial bladder cancer, early-stage prostate changes, cervical cancer in situ, or another non-invasive or slow-growing cancer
- People with other medical or psychiatric conditions that the trial doctors consider would make participation inappropriate
Pancreatic cancer (PDAC) group:
- People with serious other health conditions, such as heart disease or a history of psychiatric illness, that would make a liver biopsy procedure unsafe
- People who are pregnant or breastfeeding
- People who are unable to sign a written consent form
- People with pancreatic cancer that has spread to the liver who are already enrolled in another research trial that also involves analysing liver tumour tissue samples

Definition of tumor mutational burden, epigenetic and gene expression profile of the CRC and PDAC metastatic liver at bulk and at single cell level; Characterization of the immune landscapes of CRC and, if possible, PDAC liver MTS by high dimensional flow cytometry; Histological validation of the molecular results obtained in 1. and 2.; Definition of the antigenic landscape and TCR repertoire of CRC and PDAC liver MTS
