Who might be able to join this trial:

  • People who have had at least one blood pressure reading showing a top number of 130 or higher, or a bottom number of 80 or higher, within the past 24 months
  • People who have had a visit with their primary care doctor within the last 24 months
  • People who can read and speak English or Spanish
  • People who are 70 years of age or older
  • People who are able to understand the study and are willing to sign a consent form agreeing to take part

Who may not be able to join:

  • People whose blood pressure has consistently been below 130/80
  • People who have dementia, Alzheimer's disease, or a significant neurological (brain or nervous system) condition
  • People with serious and unstable heart conditions, such as acute heart failure, acute coronary syndrome, cardiac arrest, or who have had a liver or kidney transplant
  • People who are under 70 years of age
  • People who are unable to read or speak English or Spanish
  • People with stage 5 chronic kidney disease or end-stage kidney disease
  • People currently receiving chemotherapy
  • People whose doctors determine that taking part could put their safety at risk or that they would be unlikely to follow the study requirements
  • People whose life expectancy is estimated to be less than one year
