Who may be able to join

  • Adults aged 18 years or older.
  • People whose overall health and ability to carry out daily activities falls within a certain range, as rated by a standard medical scale called the ECOG scale (scores 0–2, meaning fully active to capable of some self-care).

  • People whose metastatic spots are visible on scans and are considered suitable for high-dose, targeted radiation treatment.
  • People who have already received ablative radiation, surgery, or radiofrequency ablation (RFA) for metastatic sites, as long as the total number of metastatic sites (including those already treated) is 5 or fewer at the time of joining the study.
  • People who have received lower-dose (palliative) radiation or RFA for painful bone metastases, as long as that treated area appears stable on imaging — or, if not stable, a treating doctor considers further high-dose radiation to be possible.
  • People who have had surgery for bone metastases followed by palliative radiation, as long as the treated site appears stable on imaging and the total number of metastatic sites is fewer than 5 at the time of joining.
  • People with blood counts measured within 15 days before joining the study that meet minimum levels: white blood cell count (neutrophils) at least 500 cells/mm³, platelets at least 50,000 cells/mm³, and haemoglobin at least 8.0 g/dL (including if achieved through a blood transfusion or similar support).
  • Women who could become pregnant must have a negative pregnancy test (blood or urine) within 14 days before joining the study.
  • People who are willing to provide written informed consent and are able to attend the required follow-up appointments.

Who may not be able to join

Each point below is a reason the trial team may not be able to accept someone. It is not a list of requirements to meet.

  • People whose cancer has continued to grow or spread during their initial systemic treatment.
  • People whose lung cancer is driven by specific gene mutations (oncogene driver mutations) (confirm with trial site for which mutations apply).
  • People with more than 5 sites of cancer spread in the body.
  • People with any individual metastatic tumour larger than 5 cm in size.
  • People with more than 3 metastatic spots within a single organ.
  • People whose primary lung tumour is not considered suitable for definitive radiation treatment.
  • People whose metastatic sites are not considered suitable for high-dose ablative radiation.
  • People with cancer that has spread to the lining of the abdomen (peritoneal disease).
  • People with cancer cells found in the fluid around the lungs (malignant pleural effusion).
  • People with cancer that has spread to the lining of the brain and spinal cord (leptomeningeal disease).
  • People with cancer that has spread to the brain stem.
  • People with evidence of spinal cord compression, or metastases located within 2 mm of the spinal cord as seen on an MRI scan.
  • People with serious, unstable heart conditions — including unstable chest pain or heart failure requiring hospitalisation in the past 6 months, or a full-thickness heart attack in the past 6 months.
  • People with a severe lung condition (such as COPD) that has recently required hospitalisation or that would prevent participation in the study.
  • People who have previously received radiation treatment to the chest area.
  • People who have had another cancer diagnosed within the last 3 years.
  • People who are currently pregnant.
