Who might be able to join this trial:

  • People who are 18 years of age or older at the time their information is reviewed

  • People under 18 years of age
  • People who have been diagnosed with end-stage kidney failure and are currently receiving dialysis treatment
  • People who have a serious medical condition that either significantly shortens life expectancy or requires ongoing active medical management (for example, cancer)
  • People who have a cognitive impairment or another condition that would prevent them from taking part in the trial's activities
  • People who plan to leave their current medical practice or move out of the local area within the next 24 months
  • People who no longer consider one of the participating sites to be their main place of care
  • People who are unwilling to give their formal consent to participate
