Who might be able to join this trial:
- Adults aged 18 or older
- People who have a type of irregular heartbeat called atrial fibrillation (AF) that is either persistent (lasting a long time without stopping on its own) or long-standing persistent (lasting more than a year)
- People whose left atrium (one of the heart's upper chambers) is at least moderately enlarged
- People whose heart is pumping with less than 50% of its normal efficiency (a measurement called the ejection fraction)
- People who are medically suitable for either of the procedures being studied in the trial
Who may not be able to join:
- People whose AF or heart failure has not yet been fully managed through available medications or lifestyle changes
- People who are unable to provide written consent to participate
- People who have had open-heart surgery in the past
- People with an active infection, or certain conditions affecting the food pipe (oesophagus), such as ulcers, narrowing, or enlarged veins
- People who have previously had a catheter ablation procedure specifically for atrial fibrillation (prior ablation for other heart rhythm conditions may still be acceptable — confirm with trial site)
- People who cannot take blood-thinning medications, or who have a blood clot in the left atrium that has not resolved despite blood-thinning treatment
- People with severe disease affecting the heart valves
- People with unstable coronary artery disease (a condition affecting the blood supply to the heart)
- People with an uncontrolled irregular heart rhythm originating from the lower chambers of the heart
- People who have had a heart attack or stroke within the past 90 days
- People who are pregnant, breastfeeding, or women of childbearing age who plan to become pregnant within six months
- People with a serious existing medical condition, or an implanted medical device, that would prevent participation in the trial procedures
