Who might be able to join this trial:
- People who receive care at one of four participating health care centers or OSF HealthCare, based on their insurance type
- People whose health insurance is Medicaid, or who are covered by both Medicare and Medicaid
- People who have been diagnosed with at least one of the following conditions: high blood pressure, diabetes, or are currently pregnant
- People who are able to read and understand English, or can read materials translated into another supported language (such as Spanish)
- Young people under 18 years of age who are pregnant and are already enrolled in the MIC Program
Who may not be able to join:
- People who are not enrolled in the MIC Program
- No one will be excluded based on their sex or race

Change in participant's blood pressure control at three points in time; Change in participant's glycemic control at three points in time; Change in Patient Assessment of Chronic Illness Care scale (PACIC) from baseline to exit from program; Change in Patient Perception of Health Care Management (adapted form of PACIC to reflect pre/postnatal care) from baseline to exit from program; Patient satisfaction, 1-item researcher generated question
