Who might be able to join this trial:

• You are 18 years of age or older and have been diagnosed with malignant pleural mesothelioma (a type of cancer affecting the lining of the lungs), confirmed by a tissue biopsy that also identified the specific type
• Your cancer has not spread to distant parts of the body, and lymph node involvement is limited or absent
• You and your medical team have agreed to monitor the cancer closely for now, rather than starting drug treatment straight away
• You are in good general health and able to carry out everyday activities with no more than minor limitations (confirm with trial site)
• Your cancer is only affecting one side of the chest, based on a CT scan
• Your lungs are working well enough to meet specific breathing test thresholds (confirm with trial site)
• You are willing and able to travel to one of two specialist radiotherapy centres (UCLH in London or The Christie in Manchester) if assigned to the treatment group
• You are willing to attend follow-up appointments at a local trial site
• Your medical team believes you are likely to be able to complete the radiotherapy planning process

Who may not be able to join:

• Your cancer has spread to other parts of the body or to the other side of the chest
• Your diagnosis was based only on fluid analysis rather than a tissue biopsy, or the cancer type has not been clearly identified
• You have previously had radiotherapy, chemotherapy, or immunotherapy for this cancer
• You have previously had major surgery to remove the affected lung lining or lung for this cancer
• You and your medical team feel that drug treatment or surgery needs to start immediately, rather than taking a watch-and-wait approach
• Your cancer has spread to lymph nodes on the other side of the chest or in the neck area
• Your cancer has grown into the heart muscle
• Your cancer has spread to lymph nodes further afield or to distant parts of the body
• You have a new fluid build-up around the lung that cannot be drained
• Your general health and ability to carry out daily activities are significantly limited
• You are currently pregnant or breastfeeding
• You have or have had another cancer that may affect your life expectancy
• You have a pacemaker or an implanted heart device called a cardioverter-defibrillator
• You have a significant lung scarring condition (mild or incidental findings may be acceptable — confirm with trial site)
• You have a serious non-cancer health condition where your estimated chance of surviving three years is less than 20%
• You have previously had radiotherapy to the chest or abdomen for a cancer and this was not discussed with the trial organisers before you were considered for the study
