Who might be able to join this trial:
- People who are male or female, from any racial or ethnic background, and between 18 and 65 years old
- Military service members receiving care at NMCSD, or civilians receiving care at UCSD Health
- People who have a history of mild traumatic brain injury (as defined by specific military and veterans' guidelines), where the injury occurred more than 3 months before joining the study
- People who are currently experiencing a Major Depressive Episode as part of Major Depressive Disorder, confirmed by a structured clinical interview
- People whose depression is rated as moderate to severe based on a standard depression rating scale (a score of 18 or higher on the HAMD-17)
- People who have been on stable psychiatric medications for at least 6 weeks, with no changes to those medications expected during the study
- People who have no safety concerns that would prevent them from receiving TMS (transcranial magnetic stimulation), based on a standard safety screening questionnaire
- People who have no safety concerns that would prevent them from having an MRI scan, based on a standard safety screening questionnaire
- People who are able to attend all required treatment appointments
- People who are able to complete all study assessments in English
- People who are able to make their own decisions and can give informed consent to participate voluntarily
Who may not be able to join:
- People who have a history of moderate, severe, or penetrating traumatic brain injury
- People who have a history of certain other neurological conditions unrelated to brain injury, such as stroke, Parkinson's disease, multiple sclerosis, epilepsy, dementia, brain surgery, cerebral aneurysm, or certain other brain or nervous system conditions (confirm with trial site for the full list)
- People who have implanted medical devices such as a pacemaker, medication pump, aneurysm clip, cochlear implant, or any metal object in or near the head that cannot be safely removed
- People who are currently experiencing a manic or psychotic episode, confirmed by a structured clinical interview
- People who currently have a substance use disorder, confirmed by a structured clinical interview
- People who are currently experiencing active thoughts of suicide or harming others
- People who are pregnant, planning to become pregnant during the study period, or currently breastfeeding
- People whose sensory or other health conditions would prevent them from taking part in the assessments or treatment
- People currently taking lorazepam at a dose of 2 mg or more per day (or an equivalent dose of a similar medication), or any anti-seizure medication, as these may reduce the effectiveness of the treatment being studied
- People currently taking medication known to lower the seizure threshold (confirm with trial site)
- People who have previously had treatment with rTMS, iTBS, or electroconvulsive therapy (ECT) that did not work for them
- People who have completed more than 4 sessions of cognitive rehabilitation (a type of brain training therapy) within the last 3 years
