Who might be able to join this trial:
- Adults aged 18 or older who are seen as outpatients (not currently admitted to hospital)
- People who have been diagnosed with type 1 diabetes by a doctor
- People who manage their diabetes using an insulin pump or by taking multiple insulin injections each day
- People whose recent blood sugar control measure (HbA1c) was 7.5% or higher, based on a lab test or continuous glucose monitor report from the last 4 months
- People who have access to a mobile phone, computer, or tablet with a video camera
- People who have had at least one visit in the past 6 months with a certified diabetes educator at one of the participating diabetes clinics (including patients transitioning from certain diabetes programs)
- People who have Ontario Health Insurance Plan (OHIP) coverage
- People who currently use an active email address, or are willing to set one up
- People who have reliable and consistent access to the internet
- People who are willing and able to attend both in-person and online appointments over a 6-month period
Who may not be able to join:
- People diagnosed with any type of diabetes other than type 1 diabetes
- People who are unable to use a computer, tablet, or mobile phone
- People who are currently pregnant
- People who are currently on dialysis (kidney treatment)
- People who are not able to speak or read English fluently (based on self-reporting)
