Who might be able to join this trial:
- People aged 65 or older who are seen as outpatients (not currently staying in a hospital)
- People who have both a regular general practitioner (GP) and a pharmacist who work together as a pair for their care
- People who have been prescribed an anti-anxiety or sleep medication from the benzodiazepine family at least 4 times in the past year

- People who do not have enough independence to carry out the steps required by the study
