Who might be able to join this trial:
- People with a general health status rated as good to moderately functional (ECOG score of 0, 1, or 2)
- People with rectal cancer that has been confirmed through tissue testing (biopsy)
- People whose cancer meets specific staging criteria — either the tumour has grown into surrounding tissue (cT3–cT4), or any tumour size with spread to nearby lymph nodes, with no spread to distant parts of the body
- People whose tumour can be surgically removed, or is expected to be removable after chemoradiotherapy
- People whose treatment plan — chemotherapy followed by chemoradiotherapy before surgery — has been agreed upon at a specialist multidisciplinary team meeting

- Women who could become pregnant must have a negative pregnancy test before joining
Who may not be able to join:
- People with diabetes that is not well controlled
- People who cannot have surgery for any reason
- People who cannot have an MRI scan for any reason
- People who cannot have a PET scan for any reason
- People who cannot receive chemotherapy for any reason
- People who have previously had radiotherapy to the pelvic area
- People with serious existing health conditions that could prevent treatment, or who have an active HIV infection or chronic hepatitis B or C
- People whose rectal cancer is of a specific type called colloid (mucinous) adenocarcinoma
- People whose cancer has already spread to distant parts of the body
- People who cannot tolerate specific chemotherapy drugs: 5-Fluorouracil, oxaliplatin, or irinotecan
- People with a known condition called Gilbert's disease (a liver condition affecting how the body processes certain substances)
- People with a known genetic variation called UGT1A1 (confirm with trial site)
- People with a known deficiency of an enzyme called Glucose-6-Phosphate Dehydrogenase (G6PD)
- People with a history of ongoing diarrhoea or inflammatory disease of the colon or rectum
- People with a history of chest pain (angina) or heart attack
- People with an active or worsening infection, or any other serious medical condition that could interfere with treatment
- People with another current cancer, or a past cancer history — except for certain previously treated skin or cervical cancers (confirm with trial site)
- People already enrolled in another clinical trial testing an experimental treatment
- Women who are pregnant or breastfeeding
- People who are legally unable to give their own consent, or who are under guardianship or have restricted freedom
- People with psychological, family, social, or geographical circumstances that could make it difficult to follow the trial schedule
