Who might be able to join this trial:
- Adults aged 18 or older
- People who have been diagnosed with pancreatic adenocarcinoma (a specific type of pancreatic cancer), confirmed by a tissue or cell sample
- People whose pancreatic cancer cannot be surgically removed, either because it has spread to nearby areas or to other parts of the body
- People who are not suitable for, or cannot tolerate, a full-strength combination chemotherapy treatment
- People who are suitable for either full-dose gemcitabine (a chemotherapy drug) alone, or a lower-dose combination chemotherapy
- People whose general health and ability to carry out daily activities meets a certain minimum level (confirm with trial site)
- People who have a tumour that can be measured on scans, or one that cannot be measured but is still detectable
- People whose blood counts (infection-fighting cells and platelets) are at adequate levels within 2 weeks before joining
- People whose liver and kidney function are within acceptable ranges within 2 weeks before joining
- People where any side effects from previous chemotherapy, including nerve-related effects, have largely resolved
- People who have given written and verbal agreement to take part, in line with local ethics requirements
- People who are capable of having children must be using appropriate contraception
Who may not be able to join:
- People whose cancer may be suitable for chemotherapy aimed at shrinking the tumour before surgery, local destruction, or radiation treatment
- People who have already had chemotherapy for their pancreatic cancer (though people who had chemotherapy after surgery and whose cancer came back more than 6 months after finishing that treatment may still be considered eligible)
- People who have another active cancer that is not being treated with curative intent, in addition to their pancreatic cancer
- People who are currently receiving any other cancer treatment
- People who are pregnant or breastfeeding
- People who clearly intend to leave the trial if not assigned to their preferred treatment group, or who cannot attend regular follow-up appointments due to personal, social, or location-related reasons
- People with any other medical condition or treatment that, in the treating doctor's judgement, could put them at risk or affect how the trial runs
- People with a known allergy or intolerance to any of the drugs used in this trial: gemcitabine, S-1, or nab-paclitaxel
