Who might be able to join this trial:
For patients:
- People aged 65 years or older
- People living with multiple health conditions — specifically, diabetes or high blood pressure, plus at least one other condition from this list: arthritis (osteoarthritis or rheumatoid arthritis), asthma, irregular heartbeat (atrial fibrillation), mild Alzheimer's disease or related memory conditions, cancer, chronic obstructive pulmonary disease (COPD), depression, heart disease, heart failure, high cholesterol, osteoporosis, kidney disease, or stroke — people with both diabetes and high blood pressure also meet this condition
- People who have had two or more visits to their clinic in the past 12 months
- People who plan to continue receiving care at the same clinic with the same doctor for at least the next 6 months
- People who do not have vision or hearing problems that cannot be corrected with glasses, hearing aids, or similar supports
- People who have not recorded a clinic visit for personal use in the past 6 months (confirm with trial site)
For caregivers:
- People who agree to their role as a caregiver for a participating patient
- People who speak English or Spanish
- People who have the ability to give informed consent to take part in research
- People aged 18 years or older
Who may not be able to join:
For patients:
- People who do not have the mental capacity to give consent to participate
- People diagnosed with schizophrenia, other psychotic disorders, or a current substance use disorder
- People with uncorrected hearing or vision problems that cannot be supported or accommodated for the purposes of the study
- People currently living in a skilled nursing home or hospice care
- People identified as having significant cognitive impairment based on a short memory screening test (a score of 4 or below on the six-item screener)
- People without access to the internet
- People who do not speak English or Spanish
- People who do not have a personal email address, do not have an email shared with a family member or carer, and are not willing to create one, as this would prevent engagement with the study activities
- People who are unwilling to create an online patient portal account if they do not already have one, as this would prevent engagement with the study activities
For caregivers:
- People who do not have the mental capacity to give consent to participate
- People currently living in a skilled nursing home or hospice care
- People without access to the internet
- People who do not speak English or Spanish
