Who might be able to join this trial:

• Adults aged 18 years or older
• People who have been diagnosed with a pancreatic cyst (known as a pancreatic cystic neoplasm) confirmed through specific specialist procedures, including fluid testing and/or imaging techniques performed during an endoscopic ultrasound
• People whose pancreatic cyst measures at least 2 cm across on a CT scan, MRI, or endoscopic ultrasound, and shows features that doctors consider concerning based on internationally recognised medical guidelines (2017 revised Fukuoka Guidelines)
• People who are not suitable candidates for surgery — this may include those with liver cirrhosis, those aged 75 or older, those with severe obesity, those with significant heart or lung conditions, those with other serious health conditions that make surgery too risky, or those who have chosen not to have surgery
• People who have a life expectancy of at least one year
• People who are able to give written consent themselves, or who have a legally authorised representative who can consent on their behalf
• Women who could potentially become pregnant must have a negative pregnancy test on the day of treatment (this is standard practice for anyone having an endoscopy with sedation)
• People who prefer non-surgical treatment after discussing options with a specialist liver, pancreas, and bile duct surgical team
• People who are not surgical candidates and who have already had a previous attempt at treating their pancreatic cyst using chemotherapy injected during an endoscopic ultrasound, under a specific existing study at Ohio State University (confirm with trial site)

Who may not be able to join:

• People whose cyst has been confirmed to be non-cancerous and non-neoplastic (such as a pseudocyst) through specialist testing
• People whose pancreatic cyst has been confirmed to be cancerous, including those with evidence of adenocarcinoma, invasive cancer, or cancer that has spread to other parts of the body
• People whose cysts or neuroendocrine tumours are located near or involving blood vessels, the bile ducts, or the main pancreatic duct, where the treatment could damage these structures
• People who have had acute pancreatitis (sudden pancreas inflammation) within the 4 weeks before the planned procedure
• People with severe or uncontrolled serious illness, or abnormal test results, that the study doctors consider would make participation unsafe
• People with a psychiatric condition that makes it impossible to reliably give informed consent
• People who are pregnant or breastfeeding
• People with a very poor general health and functioning status, classified as ECOG performance status 4 (confirm with trial site regarding what this means in practice)
• People for whom general anaesthesia is considered unsuitable, following review by the relevant assessment clinic
• People who have a cardiac implantable electrical device (such as a pacemaker or defibrillator)
