Who might be able to join this trial:

• You have been diagnosed with multiple separate colorectal cancers occurring at the same time, confirmed by a tissue sample (biopsy)
• Lab tests on at least one of your tumors show a specific genetic feature called "mismatch repair deficiency" (dMMR) or "high microsatellite instability" (MSI-H), which affects how your cancer cells repair DNA
• Your cancer is at a certain stage (T3 or T4, without distant spread), based on imaging scans — meaning the cancer has grown into or through the bowel wall but has not spread to distant parts of the body
• You have had specific scans to stage your cancer, including CT scans of your chest, abdomen and pelvis, and a high-resolution MRI scan
• You do not have a blocked bowel, or if you did, it has been relieved by a temporary bowel diversion surgery (colostomy)
• You have never had surgery on your bowel or colon before
• You have never had chemotherapy or radiotherapy before
• You have never received biological therapies (such as certain antibody treatments) or immunotherapy drugs before
• You have signed a form giving your informed consent to take part

Who may not be able to join:

• You have a heart rhythm problem that needs certain medications to control it, significant heart disease, a heart attack within the last 6 months, or moderate-to-severe heart failure
• You have high blood pressure that cannot be controlled with medication
• You have HIV, or you currently have an active hepatitis B or C infection
• You have active tuberculosis (TB), are currently being treated for TB, or were treated for TB within the past year
• You currently have another serious active infection
• Your cancer has spread to areas outside the pelvis (distant spread confirmed by scans)
• You have serious problems with your blood or organ function
• You have previously had radiotherapy to your pelvis or abdomen
• You have epilepsy that requires ongoing treatment
• You have been diagnosed with another type of cancer within the last 5 years
• You have substance misuse issues, or mental health or social conditions that could affect your participation or the reliability of the study results
• You have an active autoimmune disease (where your immune system attacks your own body) or a history of one — for example, certain lung, eye, gut, liver, kidney or thyroid conditions, or asthma requiring specific inhalers (confirm with trial site)
• You have received any vaccine against an infection in the 4 weeks before joining the trial
• You regularly take immune-suppressing medications or steroids above a certain dose
• You are known to be allergic to any of the drugs used in this trial
• You are in any unstable health condition that could put you at risk or affect your ability to take part safely
• You are pregnant, breastfeeding, or able to have children but not using contraception
• You do not wish to sign the informed consent form
