Who might be able to join this trial:
- You are between 18 and 75 years old
- You have had a local surgical removal of a rectal cancer (adenocarcinoma) using one of several specific procedures (such as TEM, TAMIS, EMR, ESD, or polypectomy — confirm with trial site if unsure which applies to you)
- Your tumor meets specific size and tissue characteristics: either it is between 3–5 cm with certain concerning features under the microscope (such as poor cell appearance, spread into blood/lymph vessels, or nerve involvement), or it is smaller than 3 cm with at least one of those features (confirm details with trial site)

- Your heart, lungs, liver, and kidneys are healthy enough to tolerate surgery

- You have, or previously had, another type of cancer
- You are not able to receive chemotherapy, radiation therapy, or surgery if needed later
- You have a history of inflammatory bowel disease (IBD) or a condition called familial adenomatous polyposis (FAP)
- You have recently been diagnosed with another cancer
- Your overall physical health is rated as very poor based on standard medical scoring scales (confirm with trial site)
- Your liver, kidneys, heart, or lungs are not healthy enough to tolerate surgery, or you have serious blood clotting problems or other major health conditions
- You have a history of severe mental illness
- You are pregnant or breastfeeding
- You have other medical or test result conditions that the trial team decides make you ineligible (confirm with trial site)
