  • You have been officially diagnosed with type 1 or type 2 diabetes for at least 1 year

  • You have a history of, or currently have, a heart or cardiovascular condition — including chest pain (stable or unstable), heart attack, mini-stroke (TIA), stroke, heart failure, significant irregular heartbeat, or significant problems with your heart's electrical system
